Dear Families,
In preparation for the new school year, we ask all families to complete updated medical information for their child(ren) to help us maintain a safe, healthy campus and meet Illinois state requirements. As we’ve previously shared, we are transitioning to a new, secure platform called August Schools to collect and store student medical records and forms. While you have only shared some information through Blackbaud in the past, we ask that families now submit complete, up-to-date medical information through this new system.
Soon, you will receive an email prompting you to log into Blackbaud to complete a short checklist:

You can access August Schools anytime via the Panther Portal. You’ll only need your Blackbaud login credentials.
In August Schools, you’ll find forms for immunizations, medications, allergies, and a brief health history. If your child has a condition requiring medication or an action plan (e.g., asthma, allergies), forms must be resubmitted each year. All medical forms must be completed by August 10, 2025. Students without a completed form will not be permitted on campus on the first day of school.

Q. What do I need to complete the form?
Have your child’s doctor's information, insurance information, and a copy of their most up-to-date immunization record. Many of the forms will require a doctor’s visit.
Q. What if my child’s allergy or medical condition does not show up?
Please select “other” and then use the free text box to enter the allergy or condition.
Q. If I am filling out a medication form, do I also need to submit a paper copy when I drop off the medication?
No. All you will need to bring is the medication in its original bottle. We will be using an electronic medical record going forward and will verify the information on the electronic form submitted at medication drop off day.
Q. Can I start and stop my August Schools medical forms?
Yes, your progress will be saved, and you can return to the form at a later time.
Step 1
Log into Blackbaud (the Panther Portal) by clicking this link, or by following the link in your email. If you need help logging in, contact. It.support@lfcds.org.
Click the “Checklist to Review” Notification. This will open a page where you will see the available checklists for you to complete.
Step 2
Click Fill Out to begin the Emergency Contact Update. This will open a form where you can share emergency contact information with us. Exisiting emergency contact information will be prepoluated. If nothing has changed, you can just click submit.
Step 3
Returning to the Checklists, to complete the Medical information, as well as to review and download forms that you may need to have filled out by a physician, Click on the text “Click here to complete the medical, travel, and consent forms.” This will take you to August Schools.
Click on Requests. You should see one for each of your children enrolled at LFCDS. Click on the request to review and begin filling out the forms. This will take you to the list of forms that must be completed for reenrollment.
Step 4
Fill out the forms in order.
Though you can go to any form on the list, it’s advisable to go through them in order, as many of the responses in the earlier forms will impact how later forms on the list will populate. For example, only medications listed in the Medication Form will be available to select when answering questions about allergies in the Allergies form. Or, the Action Plan forms will not appear if you don’t indicate that your child has a condition that requires one in the General Health form.
As you get through the forms and need to August Schools after collecting details from your doctor, you can go back to the specific form in question to update those details.
Step 5
5Some of the forms may require a signature to indicate your consent. You can sign with your finger or a stylus on a touchscreen device, or you can use a mouse to sign digitally. You can also click the button labeled “read more” to see the details of the consent release request.
Step 6
Some of the forms may require you to (1) download a PDF to print and bring it to your child’s pediatrician. Once completed, you’ll (2) upload that form.
When you are finished, all of the forms will have a green checkmark icon, and Blackbaud will show the step in the check list as complete.
